Depends on your local laws. Where my car used to be registered, I didn't have emissions testing.
Now where I am registered we just have visual and computer testing. They plug my car into a computer via the OBD2 port, and make sure no emissions related codes apply (such as P0420). Then when they are looking over the car, they just make sure they see everything that should be there.
I have a gutted cat so it visually is still there, and the P0420 code doesn't trip in my PCM due to tuning.

In my location, all pre-96 cars must go on the rollers and they do a tail pipe test. 96 and up, they hook to the computer. But let's say you reset the "readiness" status in the PCM (such as replacing a battery), if you have not failed by the computer check, they are willing to put it on the rollers (once you fail a computer scan, you MUST pass by the computer).
And on 96 and up cars, a CEL means auto fail. But I've had pre-96 cars with CEL pass.
I believe a lot of it was getting out of ODB-I.
I find it funny they test these cars this way, but I don't make the rules.

I run a cat delete and an O2 emulator/simulator for my 97 Monte. The simulator sends fluctuation of voltage between .1VDC - .9VDC, and the signal has to be different than the upstream O2 sensor. If the signals are the same, or very close to the same, then the computer thinks that the CAT is not working and it trips the SES or CEL light. I think there's plenty of threads on here about O2 emulators and CAT deletes. 3800 performance has an emulator for 2000 - 2003 monte carlos. Specifically says will not work on 2004 models.
Maniac: What change do you think could have happened where the sensor doesn't work in 2004 models? Did the computers look for tighter values or criteria?
Maybe a CAT delete in 2004+ models will require computer reprogramming.

Honestly, I am not sure what changes were made that prevent the '04+ models from working with the O2 emulator.
I do know you can delete the code from the PCM (this is different from clearing the code). Deleting the code from the PCM prevents the PCM from running that diagnostic and ever popping that condition. To my knowledge this is not detected by emissions checks.
Keep in mind, if you delete the cat and your emissions check station does a visual, they can fail you for tampering with the vehicle.

Is it worth it? Only if your cat is currently clogged and gutting it is cheaper than replacing it. If nothing is wrong with your cat system then it's not worth it IMO
