sounds to me like the fan is messing up or the t-stat for the temp on the fan is they have a temp regulator on them so they know when to turn on and off if it dont turn on in time it will cause you to heat up till it decides to turn on.
you can have a dealer check it out for you if you have warrenty . if not the only other option would be go to a bone yard and get one and replace it.
good luck.
the nosie might be the fan berings draging to.
mine runs just under 1/2 ALL THE TIME
then the stock t-stat was in it. now it runs lower then that but i have a cooler t-stat in there now.

RE: How hot should your engine get?
these cars PCM's are programed like this:
first fan starts around 185-190*
second fan starts around 190-200*
this is fine cause your Thermostat factory is a 195* unit. and middle on the gauge is about 180*
so yeah middle is fine. if you drive around with the heater or AC on, this will tell the car to kick the second fan on early. and will keep the temp down in the first quarter.
if you want to keep the car in the lower temp all the time, you have a few options:
1) leave heat or ac on all the time
2) run a relay and rig the second fan to the first
3) but the thrasher kit for the fans (basicly just a prewired relay kit)
4) finally you can buy a DHP chip or power programer and reset the fan turn on points... this is what im going to do
