Default High/Low beam issue

I will try to provide all information that I can. Any misspellings I will accredite to my phone.
2001 LS 176k miles
To start, I had DDM hid, worked like a charm. Bulbs/color began to fade so I replaced with new bulbs.
List of issues in order of appearance:
1 Low beams would become intermittent on/off and would have to use my headlight/dimmer switch to get them to work. Luckily, I had an extra switch for troubleshooting purposes. Still the problem remains.
2 Normally, you can pull back on the switch and have both high and low beams simultaneously. Soon I could no longer do that, however, I could switch solely to my high beams but not have both at the same time.
3 Eventually, I lost HID on both sides and being I deliver at night I went and purchased standard bulbs and all was well.
4 While troubleshooting the HID issue yes I checked ground and so forth, even used a new HD harness. Still only gaining power to 1 side pending how power was hooked up.
5 Upon giving up, due to not finding a solution, my current problem is now in place.
A) No high or low beam passenger side, driver side works fine. Lights are on, as if it is the light sensor, all the time while driving. See pictures attached.
Where is my issue? I have not purchased a switch or relay as of yet. I would like to hopefully fix it the first time.
