Well logic says that most of the pipes are round and that would mean a Round Cover.
However I have seen a Square/rectangular one.
If you saw the Movie Die Hard 2 where Bruce Willis tries to climb out of the man hole cover onto the runway before the bad guy lands the Plane a Herkery bird cargo plane I think. Anyway he made it.

Speaking as a professional plumber, I can tell you the reason manhole covers are round is so they can't fall through the hole. If they were square they could fall through if tilted.
