6th Gen ('00-'05): Rough idle for 5 min on start up

Default Rough idle for 5 min on start up

2002 Monte Carlo
Ok help me out with this one... what would cause a car to run poorly for the first 5 min on start up when engine is cold (cold as in not at normal operating temperature not as in its outside and its - 50) then after that it runs fine.
Notes
Replaced MAF
cleaned throttle body

Well the few things that I usually check are.
Vacuum line is off someplace? this was off on my SUV with a 3400 motor and it was hidden under a foam protector hose.
Vacuum line is leaking?
Fuel system needs cleaning?
Air filter is plugged?
Fuel Filter is plugged?
Plug wire is bad?
Needs new plugs?
These are the cheaper repairs.
